John E. Croghan, MD

“I started Dedication Health to offer innovative, personalized primary care medical service and convenience to my patients. The high-volume and impersonal nature of today’s medical system simply does not offer what each patient fully deserves.”

Dr. John E. Croghan is the founder of Dedication Health and is triple board-certified in Internal Medicine, Rheumatology, and Geriatric Medicine. He earned his medical degree at Rush Medical College, completed his Internal Medicine residency at Loyola University Medical Center, and pursued fellowship training in Geriatric Medicine at Johns Hopkins University School of Medicine and Rheumatology at George Washington University Medical Center. His broad expertise supports personalized primary care that considers prevention, chronic conditions, rheumatologic needs, and healthy aging together.

Philip H. Sheridan Jr., MD

“It is very gratifying seeing patients understand and appreciate the care we provide for them. Often we are told in their prior encounters with the “healthcare system” they felt like their issues did not matter in a personal way to the teams taking care of them. In our practice, they feel cared for like each one of them individually matters to us on a personal level, and of course this is the case. Dr. Philip H. Sheridan Jr. earned his medical degree from Loyola University Stritch School of Medicine, where he also completed his residency in Internal Medicine. After an internship at Rush Presbyterian St. Luke’s Medical Center, he returned to Loyola for fellowship training in pulmonary and respiratory disease. Dr. Sheridan is board-certified in Internal Medicine, Pulmonary Disease, and Critical Care Medicine, giving patients access to a primary care physician with valuable experience in respiratory health and the management of complex illness.

Richard D. Stern, MD

Dr. Richard D. Stern is a board-certified internal medicine physician with extensive experience in private practice. He earned his bachelor’s degree from Washington University in St. Louis and his medical degree from Wright State University. He completed his Internal Medicine residency at Evanston/Glenbrook Hospitals, where he served as Chief Medical Resident, and is a senior attending physician within the NorthShore University Health System. Through concierge medicine, Dr. Stern is able to offer the accessibility, responsiveness, and time needed to answer patients’ questions, understand their individual health goals, and provide highly personalized care.

Santiago A. Candocia, MD, FACP

Dr. Santiago A. Candocia earned a bachelor’s degree in Biological Sciences from Northwestern University and his medical degree from the University of Illinois College of Medicine. He completed his Internal Medicine residency at Northwestern University Medical School, served as Chief Resident in Medicine at Evanston Hospital, and held Clinical Instructor roles at Northwestern and the University of Chicago. Dr. Candocia is board-certified in Internal Medicine and also provides expertise in pre-travel consultations, immunizations, and travel safety.

Anat A. Lansky, MD

Dr. Anat A. Lansky earned bachelor’s degrees in Chemistry and Psychology from Indiana University before receiving her medical degree from St. George’s University School of Medicine. She completed her Internal Medicine residency at Advocate Illinois Masonic Medical Center in Chicago. Board-certified in Internal Medicine and specializing in Women’s Health, Dr. Lansky values accessible communication, individualized care plans, and the opportunity to build meaningful, long-term relationships with her patients.

Jana Mechurova, MD

Dr. Jana Mechurova received her medical degree from Komenskeho University in Bratislava, Slovakia, and completed her residency at John H. Stroger Hospital in Chicago. She previously served as an Assistant Program Director at Mt. Sinai Hospital, where she taught and supervised residents while providing primary care to underserved patient populations. Dr. Mechurova is board-certified by the American Board of Internal Medicine and is fluent in English, Slovak, and Czech, with basic Spanish proficiency.

Jason Robin, MD, FACC

Dr. Jason Robin is a board-certified cardiologist with expertise in Cardiology, Nuclear Cardiology, Echocardiography, Cardio-oncology, and Internal Medicine. He graduated with highest honors from Rush Medical College before completing his Internal Medicine residency, serving as Chief Medical Resident, and completing his Cardiology fellowship at Northwestern University. Dr. Robin has particular expertise in cardio-oncology and sports cardiology and currently serves as Director of Cardiology-Oncology at City of Hope. At Dedication Health, he provides comprehensive cardiovascular care ranging from prevention and early detection to the diagnosis and management of complex heart disease, with an emphasis on timely access, advanced diagnostics, and highly personalized care.

Michael J Waligora, MD, FACC

Dr. Michael J Waligora is a board-certified cardiologist with more than 30 years of experience in cardiovascular medicine. He earned his medical degree from Saint Louis University School of Medicine, completed his Internal Medicine residency at Northwestern University, and remained there for his general Cardiology fellowship. His expertise includes preventive and sports cardiology, advanced diagnostics, coronary artery disease, heart failure, hypertension, and complex heart rhythm disorders. Yolandra Johnson, MD

Dr. Yolandra Johnson is a board-certified gastroenterologist who brings decades of private-practice experience caring for patients on Chicago’s North Shore. She completed her internship and residency at Yale-New Haven Hospital, followed by a Gastroenterology fellowship at the University of Chicago. She also serves as a Senior Clinician Educator at the University of Chicago’s Pritzker School of Medicine. At Dedication Health, Dr. Johnson emphasizes timely evaluation, thoughtful communication, and highly individualized care for preventive screenings, new digestive concerns, and ongoing gastrointestinal conditions.

Jonathan Williams, DO

Dr. Jonathan Williams is board-certified in Gastroenterology and completed fellowship training in Gastroenterology, Hepatology, and Nutrition at Loyola University Medical Center. A Clinical Assistant Professor at the University of Chicago’s Pritzker School of Medicine and a multiple-time teaching award recipient, he combines clinical experience with a strong commitment to patient education. His concierge approach gives patients time to explain their symptoms fully, understand their options, and take an active role in decisions about their digestive health.

Karen Raden, MS, RD, CCN

“I am particularly passionate about helping patients understand how nutrition can support both how they feel today and how they age over time. My goal is to help each patient better understand their body, feel confident in their nutrition choices, and develop realistic strategies that support both current health and long-term well-being.”

Karen Raden is a Registered Dietitian and Certified Clinical Nutritionist with more than 25 years of experience. She provides personalized nutrition guidance for patients with a range of goals and concerns, with particular expertise in digestive health and complex medical or nutritional histories. When appropriate, she may incorporate advanced laboratory and functional testing to better understand digestive function, nutrient status, inflammation, and other factors that can inform a practical, individualized nutrition plan.
